MAJORS FLYING CLUB ENROLLMENT PACKAGE
This page contains enrollment information and an application for the Majors Flying Club.
If you are interested in becoming a member of our flying club, please follow these instructions.
- Read the Introduction to the Club, the club By-Laws and the Majors Flying Club Operating Rules. The Introduction will get you going on who we are and points of contact. Please call Dennis Mathis or any board member shown in the Introduction with questions you might have.
- Fill out the Application and, if you are already a student or licensed pilot, attach:
a. A copy of your Pilot Certificate (front and back).
b. Your last FAA flight review endorsement if required. (The full endorsement, not just the logbook entry for the flight)
c. A copy of your current FAA medical.
d. A copy of your current renters insurance policy or certificate (See policy requirements on the Membership tab).
e. Photocopies of the last 3 filled-out pages of your logbook.
- Place your name on the membership Checklist.
- Take the Open Book Quiz and include it in your application. Subjects are covered in the Operating Rules and By-Laws.
- Read and sign the Covenant Not to Sue form, and have it notarized. (If the member applying is 17 years old or younger, a parent/guardian must fill out this version for dependents).
- We prefer that you arrange with your bank to have regular monthly payments transferred to the Majors Flying Club account. Please contact the club Treasurer for the account information.
You can also pay by check monthly but it is not preferred. Manipulating checks each month causes our treasurer more work which is contrary to keeping our costs down. To pay dues by check, please pay at least 2 months worth of dues in advance at initial enrollment.
- Put the following in an envelope:
- Checklist
- Application
- Quiz
- Notarized Covenant Not To Sue
- Any copies of Pilot and Medical Certificates, renters insurance policies, and logbook pages. Be sure to include a copy of your last Flight Review endorsement.
- A check for the appropriate Initiation Fee
- A check for a minimum of two months' worth of dues
